The assignment should contain the following:

– Brief introduction of the case study

– State the problem that will be discussed for the assignment and provide evidence and rationale of the identified problem.

– Based on the selected problem, critically analyse and discuss the problem and issues faced by the client and/or family.

– Provide focused management care plan and interventions for the client and/or family. The management plan also should reflect your ability to critically analyse the problems and develop relevant and focused interventions.

– Select and discuss one (1) of the following concepts related to the client and/or family as appropriate:

• Case management

• Palliative care, or

• Care transition.

Start with a brief introduction of the case study. Keep this part concise but ensure you include the key details about the client, such as their health condition, age, and family situation. This introduction is your chance to set the context, so the reader understands the background before you dive into the problem and analysis.

2. Identify and State the Problem

Next, identify the main problem affecting the client or their family. Be specific about what issue you will focus on and why it is significant. Provide evidence and rationale for this problem—this could be based on the client’s medical history, family dynamics, or social circumstances. For instance, if you’re addressing a chronic illness, mention how it impacts the client’s daily life and the strain it puts on the family.

Use relevant references or research to back up your identification of the problem. This shows you’re not just guessing but basing your analysis on evidence.

3. Critically Analyse the Problem

Once the problem is identified, move on to a critical analysis. This means digging deeper into the issues faced by the client and their family. What are the underlying causes of the problem? What barriers or challenges are making it harder for the client to manage their situation?

For example, if the client is dealing with a terminal illness, you might analyze the emotional, financial, and physical strain on the family. Or if the client has limited access to care, discuss the systemic issues contributing to this problem.

4. Develop a Management Care Plan

Based on your analysis, create a focused management care plan. This plan should be specific to the client’s needs and address the issues you’ve discussed. For instance, if the problem is related to care transitions, your plan might include a detailed process for coordinating care between hospitals and home care providers.

Make sure your care plan is realistic and practical. It should include clear interventions, such as setting up regular home visits, connecting the family with support groups, or providing palliative care services.

5. Discuss One Relevant Concept

Finally, choose one of the three concepts—case management, palliative care, or care transition—and discuss how it applies to your client. For example, if you choose care transition, discuss how the client’s move from hospital to home care can be managed smoothly and what strategies can be used to prevent gaps in care.
