For this GBA, students need to work in their groups as a multidisciplinary team to improve the health and well-being of patients with chronic illnesses referred to them by doctors. Decide as a group to focus on patients with one type of physical chronic illness (e.g., diabetes, hypertension, hyperlipidemia, etc). Form a group of 4-5 members. Each member will take on one of the following respective specialist roles in a multidisciplinary team:
1) Stress and Mental Well Being Specialist (i.e., looks at how to manage stress management; looks at factors, e.g., positive psychology, that contribute to well-being of the patients)
2) Nurse Clinician (i.e., looks at how stress contributes to disease, looks at medication adherence and adherence to recommendations of the team and doctors)
